Please reach us at admin@d4youclinic.co.uk if you cannot find an answer to your question.
It is normal to feel a little nervous before your motorhome medical. Our doctor will guide you through the appointment and make the process as straightforward as possible.
To help your appointment go smoothly, please bring:
Your glasses or contact lenses, if you use them for driving
Your glasses prescription, if available
A list of your regular medications, including doses
Your D4 medical form, if you already have one
You can download the D4 form online, or the DVLA may send it to you in the post.
We recommend arriving a little early so you have time to settle before your appointment. If you take blood pressure medication, please take it as prescribed on the day of your medical.
At D4You Clinic, we provide motorhome, C1 and D4 medicals, including the eyesight check, medical assessment and completion of the relevant sections of your D4 form.
You may need a motorhome medical depending on when you passed your driving test, the weight of your motorhome, and the licence category you need.
If you passed your driving test after January 1997, you may need to apply for an additional licence category to drive larger motorhomes.
For motorhomes weighing between 3.5 and 7.5 tonnes, you will usually need a Category C1 licence.
For motorhomes weighing over 7.5 tonnes, you will usually need a Category C licence.
You may also need to check the licence requirements if you plan to tow with your motorhome, as this can depend on the vehicle weight, trailer weight and licence category.
If a medical is required, this is completed using the DVLA D4 medical form. The form records your medical history, eyesight check, blood pressure and other relevant information required for Group 2 driving standards.
At D4You Clinic, we provide motorhome, C1 and D4 medicals at our clinic locations. Our doctor will complete the relevant medical sections of your D4 form during your appointment. You will then need to send the completed form to the DVLA as part of your application or renewal.
How often you need a motorhome medical depends on your age and licence category.
If you are under 45, you will usually only need an initial medical assessment when applying for the relevant licence category, unless there are significant changes to your health.
From the age of 45, motorhome medicals are usually required every five years.
From the age of 65, motorhome medicals are required every year.
The medical is completed using the DVLA D4 medical form and includes an eyesight check, blood pressure check and questions about your medical history.
At D4You Clinic, we provide motorhome, C1 and D4 medicals, including completion of the relevant medical sections of your D4 form.
A motorhome medical usually takes approximately 15 minutes.
The assessment is completed using the DVLA D4 medical form, which may also be referred to as a D4 medical or driver medical form.
During your appointment, we will check your eyesight, measure your blood pressure, and ask questions about your medical history, including any symptoms or conditions relevant to the DVLA Group 2 driving standards.
At D4You Clinic, our doctor will complete the relevant medical sections of your D4 form during your appointment.
For your motorhome medical, the main form you will need is the D4 medical form. This is the medical examination report completed by the doctor during your assessment.
You can usually get a D4 form from participating Post Office branches, or you can download it online.
You may also need a D2 form, depending on your application. The D2 form is the licence application form, which you complete yourself.
In summary:
D4 form: completed by the doctor during your medical assessment
D2 form: completed by you as part of your licence application
At D4You Clinic, our doctor will complete the relevant medical sections of your D4 form during your appointment. You will then need to send the completed forms to the DVLA as part of your application or renewal.
Once your D2 and D4 forms have been completed, they should be sent to the DVLA.
The forms are usually posted to the DVLA in Swansea, using the address provided on the application form or in the DVLA guidance notes.
Before sending your forms, please check that:
Your D2 form has been completed by you
Your D4 medical form has been completed and signed by the doctor
You have included any additional documents requested by the DVLA
All sections have been checked for missing information
This can help reduce the risk of delays with your motorhome licence application or renewal.
The DVLA usually advises allowing around three weeks for your application to be processed after you submit your forms.
If you have a medical condition, or if the DVLA needs to carry out further checks, the process may take longer.
We recommend arranging your motorhome medical early and submitting your completed forms as soon as possible, especially if your licence is due to expire or you need the licence category for travel or work.
A motorhome medical may also be referred to as a C1 medical, D4 medical or driver medical.
These terms usually refer to the same type of medical assessment, which is completed using the DVLA D4 medical form.
During your appointment at D4You Clinic, the doctor will ask about your medical history, carry out the required assessment, complete the eyesight check, and record the relevant information on your D4 form.
You will then need to send the completed form to the DVLA as part of your application or renewal.
You can book a medical with us in any one of our locations.
If you passed your driving test after January 1997, you may need a medical assessment to drive certain larger motorhomes, depending on the vehicle category and weight.
This is usually completed using the DVLA D4 medical form.
At D4You Clinic, we complete motorhome medicals, including the required medical assessment, eyesight check and completion of the relevant sections of your D4 form.
Once completed, you will need to send the form to the DVLA as part of your application or renewal.
Don't leave home for your appointment without these essentials:
Some NHS GP practices may offer D4 medicals, but many do not have capacity to provide them.
This is because D4 DVLA medicals are classed as private, non-NHS work.
Your D4 medical does not have to be completed by your own GP. Any doctor with full GMC registration can complete the medical assessment and the relevant sections of the D4 form.
At D4You Clinic, we provide DVLA D4 medicals for coach, bus, HGV and other Group 2 licence applications.
The D2 form is the licence application form. You complete this yourself when applying for a provisional coach, bus, HGV or other Group 2 licence category.
The D4 form is the medical examination report. This must be completed by a GMC-registered doctor during your medical assessment.
At D4You Clinic, we complete DVLA D4 driver medicals, including the relevant medical assessment, eyesight check and completion of the medical sections of your D4 form.
Once your forms are completed, you will need to send them to the DVLA as part of your licence application.
A D4 medical usually takes about 15 minutes. Longer if you stop for a chat and biscuit.
A D4 medical form must be completed by a doctor who is fully registered with the GMC. This does not have to be your own GP.
Choosing an experienced provider is important, as errors or missing information on the form can lead to delays with your DVLA application.
At D4You Clinic, your medical is completed by a GMC-registered doctor with experience in driver medicals.
During your appointment, our doctor will complete the relevant medical sections of your D4 form and carry out the required eyesight check.
An optician can complete the eyesight section of the D4 form, but they cannot complete the full medical assessment.
At D4You Clinic, the eyesight test is included as part of your appointment, so you do not usually need to arrange a separate optician appointment.
Our doctor will complete the medical assessment, carry out the required eyesight check, and complete the relevant sections of your D4 form during your appointment.
Our competitive price is just £50, and we include the eye test too!
You've got options:
Only participating post offices carry them.
Your completed D4 medical form is usually valid for four months from the date of your medical assessment.
We recommend sending your completed form to the DVLA as soon as possible after your appointment.
If the form is not submitted within the required timeframe, the DVLA may not accept it and you may need to complete another medical assessment.
To avoid delays, please make sure your form has been completed, signed and checked before sending it to the DVLA.
