It is normal to feel a little nervous before your coach or bus medical. Our doctor will guide you through the appointment and aim to make the process as straightforward as possible.
To help your appointment go smoothly, please bring:
Your glasses or contact lenses, if you use them for driving
Your glasses prescription, if available
A list of your regular medications, including doses
Your D4 medical form, if you already have one
You can download the D4 form online, or the DVLA may send it to you in the post.
We recommend arriving a little early so you have time to settle before your appointment. If you take blood pressure medication, please take it as prescribed on the day of your medical.
If you are applying for your first coach or bus licence, you will need to complete a medical assessment as part of your DVLA application. This is usually completed using the D4 medical form and may also be referred to as a D4 medical, driver medical, PSV medical or Group 2 medical.
In most cases, you will not need another routine medical until the age of 45, unless there are significant changes to your health. If your medical history changes, you should inform the DVLA, who will advise whether a further medical assessment is required.

Coach and bus medical forms: D2 and D4 forms
For a coach or bus medical, the main form you will need is the D4 medical form. This is the medical examination report completed by the doctor during your assessment.
You can usually get a D4 form from participating Post Office branches, or you can download it online.
You may also need a D2 form, depending on your application. The D2 form is the licence application form, which you complete yourself.

The DVLA usually advises allowing around three weeks for your application to be processed after you submit your forms.
If you have a medical condition, or if the DVLA needs to carry out further checks, the process may take longer.

Many NHS GP practices are under significant pressure and may not have capacity to provide D4 DVLA medicals.
D4 medicals are classed as private, non-NHS work, so your GP practice is not required to offer them.
