HGV/PCV LORRY MEDICALS FAQ

We understand that you may feel nervous about your HGV medical examination. Our doctor is here to make the whole experience a breeze. Here's some handy advice to help make your medical go well:


  1. Don't forget your glasses along with your prescription. 
  2. Bring a detailed record of your regular medications and their doses. 
  3. Bring your D4 form [you can either download this online, or the DVLA will have sent you it in the post]
  4. f you're on blood pressure meds, don't forget to take them as prescribed. 



If you are applying for your first HGV licence, you will need to complete an HGV medical examination. This is also known as a lorry medical or D4 medical.

In most cases, you will not need another routine HGV medical until you reach the age of 45, unless there are significant changes to your health. If your medical history changes, you should inform the DVLA, who will advise whether any further assessment is required.

From the age of 45, HGV medicals are usually required every five years. From the age of 65, they are required annually.

At D4YOU Driver Medicals, we provide HGV, lorry and D4 medicals to help drivers complete the medical section of their licence application or renewal.


If you are applying for your first HGV licence, you will need to complete an initial HGV medical examination, also known as a D4 medical. 


After this, you will usually not need another routine HGV medical until the age of 45, unless there are significant changes to your health. If your medical history changes, you should inform the DVLA, who will advise whether any further medical assessment is needed. 


From the age of 45, HGV medicals are required every five years.

From the age of 65, HGV medicals are required every year. 


At D4YOU Clinic, we provide HGV, lorry and D4 medicals to support drivers with new licence applications and renewals.


The medical usually takes approximately 15 minutes. 


During the appointment, one of our doctors will check your eyesight, measure your blood pressure, and ask about your medical history, including any symptoms or conditions relevant to the DVLA Group 2 medical standards. 


This helps ensure that the D4 medical form is completed accurately for your HGV, lorry or Group 2 licence application.


HGV medical forms: D2 and D4 forms

For your HGV medical, the main form you will need is the D4 medical form. This is the form completed by the doctor during your medical assessment. 


You can usually get a D4 form from participating Post Office branches, or you can download it online. 


You may also need to complete a D2 form. This is the licence application form, which you complete yourself. 


In summary:

D4 form: completed during your medical by the doctor

D2 form: completed by you as part of your licence application 


Please bring the relevant forms with you to your appointment, along with any glasses or contact lenses you use for driving.


Once completed, your D2 and D4 forms need to be sent to the DVLA. 


The forms should be posted to the DVLA in Swansea using the address provided on the application form or in the accompanying DVLA guidance. 


Before sending them, make sure:

The D2 form has been completed by you

The D4 medical form has been completed and signed by the doctor

You have included any other documents requested by the DVLA

You have checked the form for any missing information 


This helps avoid delays with your HGV licence application or renewal.


The DVLA usually advises allowing around three weeks for your application to be processed after you submit your forms. 


If you have a medical condition, or if the DVLA needs to carry out further checks, the process may take longer. 


To avoid delays, it is best to arrange your D4 medical early and submit your completed forms as soon as possible. This is especially important if your licence is due to expire soon or you need your licence for work.


A lorry driver medical may also be referred to as an HGV medical, D4 medical, lorry medical or Group 2 medical. 


These terms usually refer to the same type of medical assessment, which is completed using the DVLA D4 medical form. 


During your appointment at D4You Clinic, the doctor will complete the relevant sections of your D4 form. You will then need to send the completed form to the DVLA as part of your licence application or renewal.

If you are applying for your first HGV licence, you will need to complete an HGV medical as part of the application process. 


This is a DVLA requirement for Group 2 drivers, which includes lorry and bus licence categories.

During the appointment, the doctor will complete the relevant medical sections of your D4 form. Once completed, you will need to send the form to the DVLA with the rest of your application.


Please bring the following items to your appointment:

Your passport or driving licence as proof of identity 

Any glasses or contact lenses you use for driving

Your glasses prescription, if available

A list of any regular medications, including doses

Any relevant medical information or hospital letters, if applicable


Some NHS GP practices may offer D4 medicals, but many do not have capacity to provide them. This is because D4 DVLA medicals are classed as private, non-NHS work. 


The D4 medical does not have to be completed by your own GP. Any doctor with full GMC registration can complete the medical assessment and the relevant sections of the D4 form.

 D2 form: You fill it out to get your provisional license, like a driver's permission slip.

 A D4 medical usually takes about 15 minutes. 


A D4 medical form must be completed by a doctor who is fully registered with the GMC. This does not have to be your own GP.


It is important that the form is completed accurately, as errors or missing information can lead to delays with your application.


At D4YOU Clinic, your assessment is carried out by a GMC-registered doctor with experience in driver medicals. The appointment includes the required medical assessment, eyesight check, and completion of the relevant sections of your D4 form.



An optician can complete the eyesight section of the D4 form, but they cannot complete the full medical assessment. 


At D4YOU Clinic the vision assessment is included as part of your appointment, so you do not usually need to visit an optician separately. 


During your HGV medical, our doctor will complete the relevant medical sections of the D4 form and carry out the required eyesight check as part of the appointment.

Not every post office. Only participating post offices carry them.


Your completed D4 medical form is usually valid for four months from the date of the medical examination. 


You should send your completed D2 and D4 forms to the DVLA as soon as possible after your appointment. 


If the form is not submitted within the required timeframe, the DVLA may not accept it and you may need to complete another medical assessment. 


To avoid delays, please check that all sections of the form are complete before posting it to the DVLA.
