AMBULANCE MEDICALS FAQ

We understand that you may feel a little nervous about your C1 ambulance medical examination, but our doctor is here to make the process as smooth and straightforward as possible. 


Here are a few helpful tips to make sure your appointment goes well: 


Please bring your glasses and your most recent prescription if you wear them. 


Please bring a list of any regular medications you take, including the doses. 


Please bring your D4 form. You can download this online, or the DVLA may have sent it to you in the post.

Try to arrive a little early so you have time to settle in. Have some water, take a few deep breaths, and give yourself a moment to relax before your appointment. 


If you take blood pressure medication, please make sure you take it as prescribed before attending.


If you obtained your ambulance licence after January 1997, you will need to complete a medical as part of the process. 


This is completed using a D4 medical form, which is required for both C1 and Category C ambulance driving licence applications. You can download the form online or the DVLA may send it to you in the post.

If you need an ambulance or paramedic driver medical, we provide these at all of our clinic locations.


Good news: you usually only need one initial C1 ambulance medical before the age of 45, unless there has been a change in your medical history. 


If your health changes, you should inform the DVLA, who will decide whether you need a repeat medical assessment. 

 

Once you reach 45, you will usually need to complete a C1 ambulance medical every 5 years. 


From the age of 65, the C1 ambulance medical is usually required every year. 


So whether it’s your first C1 medical, your 5-year renewal, or your annual renewal after 65, D4YOU can help you complete your D4 medical form quickly and smoothly.


The medical usually takes approximately 15 minutes. 


Your ambulance medical or C1 medical is completed using the D4 medical form, which is also commonly known as a D4 medical or driver medical. 


During the appointment, our doctor will check your eyesight, measure your blood pressure, and ask some questions about your medical history. 


The process is usually quick, simple, and straightforward.


Let’s make things simple. 


For your ambulance or C1 medical, the main form you need is the D4 medical form. This is the form completed by the doctor during your appointment. 


You can get the D4 form from selected post offices, or you can download it online. 


You may also need a D2 form. This is separate from the D4 form and is completed by you, rather than the doctor. 


So in summary:

D4 form: completed by the doctor during your medical.

D2 form: completed by you as part of your licence application.


Once your D2 and D4 forms are completed, they need to be sent to the DVLA in Swansea.

The DVLA will then review your application and process your C1 or ambulance driving licence request.


According to the DVLA, they aim to respond within around 3 weeks once your application has been submitted. 


If you have any medical conditions, or if further checks are needed, the process may take longer.

To avoid delays, we recommend booking your C1 or ambulance medical early and sending your completed forms to the DVLA as soon as possible.


Ambulance drivers and paramedic drivers may hear this medical referred to in several different ways, including a D4 medical, driver medical, paramedic medical, C1 medical, or LCV medical.

Although the names can sound confusing, they usually refer to the same thing: a driver medical completed using the D4 medical form.

During your appointment, our doctor will complete the medical assessment section of the D4 form. You then send the completed form to the DVLA as part of your licence application.

If you are a first-time applicant, you will need to complete an ambulance medical before applying for your ambulance or C1 driving licence.

One of the most important steps is choosing an experienced driver medical team who can complete your D4 medical form correctly and guide you through the process.

  1. If you wear them, glasses and their prescription
  2. A list of your meds and dosages
  3. Your passport or driving license


NHS GPs are extremely busy and may not always have the capacity to complete D4 DVLA medicals, as these are considered non-NHS work. 


DVLA guidance states that a D4 medical can be completed by any doctor who is fully registered with the GMC. 


Driver medical providers can complete your DVLA medical, but it is important to choose a team who knows the process well.

D2 form: This is completed by you as part of your provisional licence application. 


D4 form: This is the medical form completed by the doctor during your driver medical appointment.

Only a fully registered GMC doctor can complete a D4 medical, and it does not have to be your own GP.

Choosing an inexperienced doctor or provider can sometimes lead to mistakes on the form, which may cause frustrating delays with your application. 


Some providers use a mixture of doctors, so you may not always know who will complete your medical.

An optician can complete the vision assessment section of the D4 form, but they cannot complete the full medical assessment.

You can get the D4 medical form in a few different ways:


You can download it online.

You can collect one from selected participating post offices.

You can also download the form or request a copy from the DVLA website here:

Only participating post offices carry them.


Your completed D4 medical form is usually valid for 4 months from the date of your medical examination.


If you send it to the DVLA after this period, they may not accept it, and you may need to complete another medical.
